19. Fetale Programmierung

Zusammenfassung

Prä- und perinatale Einflüsse spielen neben der genetischen Disposition und verschiedenen Lebensstilfaktoren eine entscheidende Rolle für Gesundheit und Krankheit im späteren Leben. Ein als Fetale Programmierung bezeichneter Prozess während besonders „kritischer“ Entwicklungsphasen ist Ausdruck einer Adaptation an unphysiologische Umwelteinflüsse wie intrauterine Mangel- oder Überversorgung und fetale Stressaktivierung und führt zu einer „Fehlprogrammierung“ von Organfunktionen und Stoffwechselregulationen, auf deren Basis sich später chronische Krankheiten wie Adipositas, Diabetes mellitus, kardiovaskuläre und mentale Erkrankungen, aber auch Krebs entwickeln können. Zunehmend werden die Mechanismen epigenetischer, molekularer, zellulärer und funktionell-regulatorischer Ebene erkannt. Präventionsorientierte Konzepte können bereits während der Schwangerschaft einen ganz wesentlichen Beitrag zur Primärprävention zahlreicher Erkrankungen im späteren Leben des Kindes leisten.
