Hintergrund

Psychose ist ein nosologisch unspezifischer Begriff, und ihr können psychiatrische (z. B. Schizophrenie und bipolare Störung) sowie eine Vielzahl somatischer und exogener Erkrankungen zugrunde liegen. Psychosen können vielfältige psychiatrische und somatische Notfälle verursachen, weshalb sie für die Notfallmedizin eine anspruchsvolle Herausforderung darstellen.

Methode

Recherche und Auswertung aktueller Literatur.

Ergebnisse und Schlussfolgerung

Wenn der Notarzt alarmiert wird, ist das klinische Bild meist geprägt von einer Psychopathologie, die den Zugang zum Patienten und dessen Einsicht in die Notwendigkeit einer Behandlung erschwert. Aufgrund häufig fehlender Krankheitseinsicht ist die Anamnese- und Befunderhebung erschwert. Eine Psychose, die mit Agitiertheit oder Aggressivität einhergeht, erfordert ein umsichtiges Vorgehen und sollte Strategien der Verhaltensmodifikation, z. B. Deeskalation, ebenso umfassen wie auch den differenziellen Einsatz von Psychopharmaka. Medikamente der Wahl sind Antipsychotika und Benzodiazepine.
