Zusammenfassung

Psychiatrische Störungen in einer zentralen Notaufnahme (ZNA) sind keine Seltenheit. Bisherige Untersuchungen legen nahe, dass der überwiegende Anteil dieser Störungen nicht erkannt und nicht behandelt wird. Zur Verbesserung der diagnostischen Qualität können die meist psychiatrisch unerfahrenen Ärzte auf kurze, aussagekräftige und valide Screeninginstrumente zurückgreifen. Die Therapie psychiatrischer Störungsbilder in der ZNA sollte sich als syndromorientierte Krisenintervention verstehen, die bei gegebener Notwendigkeit auch eine längerfristige Behandlung bahnen oder initiieren kann. Dabei spielt neben psychotherapeutischen Strategien die Pharmakotherapie eine wichtige Rolle. Wir machen Vorschläge für den differenzierten Einsatz von Psychopharmaka in der ZNA. Aufgrund der vorliegenden Daten erscheint eine enge Anbindung eines Psychiaters an eine ZNA sinnvoll.
