31. Geburtsüberwachung

Zusammenfassung

Die Kardiotokographie (CTG) ist das Standardverfahren für die fetale Geburtsüberwachung. Ihr Vorteil ist die hohe Sensitivität bei der Azidoseerkennung, Nachteile stellen die schlechte Reproduzierbarkeit und die begrenzte Spezifität dar, wenn das CTG ohne flankierende Diagnostik angewandt wird. Für die weitere Abklärung wird die fetale Blutgasanalyse (FBA) empfohlen, wodurch operative Entbindungen aufgrund falsch-pathologischer CTG-Muster reduziert werden können. Der Bedarf für die FBA kann wiederum durch Anwendung der ST-Analyse des fetalen EKG signifikant vermindert werden. Der klinische Nutzen anderer Überwachungsverfahren (Pulsoxymetrie, Nahinfrarotspektrometrie etc.) konnte bislang nicht belegt werden, weshalb diese nicht als Basis für klinische Entscheidungen herangezogen werden sollten. Die „klassische“ Auskultation fetaler Herztöne ist eine personalintensive Alternative zum intrapartalen CTG, die darüber hinaus den medikolegalen Dokumentationsansprüchen nicht genügt.
