32. Geburtseinleitung

Zusammenfassung

Aufgrund verschiedener Gründe, allen voran aufgrund des steigenden mütterlichen Alters und einer entsprechenden Zunahme an Risikoschwangerschaften, ist die Prävalenz der Geburtseinleitung in den letzten 20 Jahren deutlich angestiegen und hat mittlerweile über 20 % laller Schwangerschaften erreicht. Ziel dieser Intervention ist ein besseres perinatales Outcome für Mutter und Kind als bei expektativem Vorgehen. Zur Geburtseinleitung werden heute – je nach Indikation und klinischer Situation – folgende Methoden eingesetzt: intravaginale Prostaglandine, Oxytozininfusion mit Amniotomie und intrazervikale Ballonkathetereinlage. Misoprostol ist ein sehr effizientes Prostaglandin E1-Analogon zur Geburtseinleitung und steht nun als zugelassenes kommerziell erhältliches Präparat zur Verfügung. Die Einleitung nach vorausgegangener Sectio ist eine besondere Herausforderung, da einerseits das Risiko der Uterusruptur steigt und andererseits der Einleitungserfolg abnimmt.
