33. Vorzeitiger Blasensprung am Termin

Zusammenfassung

Ein vorzeitiger Blasensprung am Termin (≥37 SSW) betrifft etwa 8–10 % aller Schwangerschaften. Als Latenzperiode wird das Zeitintervall zwischen Blasensprung und Einsetzen regelmäßiger Wehentätigkeit bezeichnet. Bei 70–80 % der betroffenen Schwangeren setzt die spontane Wehentätigkeit innerhalb von 24 h, bei etwa 95 % innerhalb von 72 h ein. Der vorzeitige Blasensprung kann meist klinisch durch vaginalen Flüssigkeits- bzw. Fruchtwasserabgang festgestellt werden. Im Rahmen der Spekulumuntersuchung ist ein Flüssigkeitsabgang aus der Zervix und eine entsprechende Flüssigkeitsansammlung im hinteren Fornix beweisend. Bei gesichertem Blasensprung sollten folgende Parameter erhoben werden: Gestationsalter, Gesundheitszustand und Lage des Fetus, Reifzustand der Zervix, Hinweise für das Vorliegen einer Infektion und Prüfung der Notwendigkeit einer B-Streptokokken-Prophylaxe. Die Geburtseinleitung nach einem vorzeitigen Blasensprung am Termin weist gegenüber dem abwartenden Verhalten Vorteile auf.
