Chronische Herzinsuffizienz im Alter

Zusammenfassung

Mit zunehmendem Alter steigt sowohl die Inzidenz der Herzinsuffizienz als auch der Anteil der diastolischen Dysfunktion im Gegensatz zur systolischen Funktionseinschränkung. Die Symptomatik ist im Alter häufiger unspezifisch und kann sich lediglich in Müdigkeit äußern, was die Diagnose erschwert. Die Einteilung und der diagnostische Weg unterscheiden sich nicht vom Prozedere bei jüngeren Patienten. Im Bereich der Therapie ist die Datenlage für ältere Patienten geringer, da multimorbide geriatrische Patienten nicht die typische Studienpopulation widerspiegeln. Trotzdem sollen ACE-Hemmer, AT1-Blocker, β-Blocker, Aldosteronantagonisten, Diuretika und Digitalisglykoside auch im Alter, in individueller Abwägung der Indikationen und Kontraindikationen sowie speziell der Interaktionen mit den übrigen Erkrankungen und Medikamenten, eingesetzt werden. Die Anzahl herzchirurgischer Patienten über 75 Jahre steigt an. Für diese Patientengruppe wird das Operationsrisiko in Scores noch häufig zu hoch eingeschätzt, obwohl nach individueller Einschätzung heute chirurgische und interventionelle Eingriffe, z. B. an den Herzklappen, mit großer Sicherheit durchgeführt werden können.
