Diagnostik des akuten Myokardinfarkts bei älteren Patienten

Zusammenfassung

Die Frühdiagnose eines akuten Myokardinfarkts (MI) verbessert sich durch Einführung der hochsensitiven Troponintests. Diese Assays können bereits geringe myokardiale Schädigungen messen, die durch konventionelle Troponinassays nicht nachweisbar sind. Insbesondere bei älteren Patienten, die einen höheren basalen Troponinwert zu haben scheinen, müssen die Ergebnisse immer im Kontext von Anamnese, körperlichem Untersuchungsbefund, EKG-Befund und ggf. weiteren Untersuchungsergebnissen beurteilt werden. Selbst geringfügige Erhöhungen des hochsensitiven Troponins zeigen ein erhöhtes Risiko für Tod oder MI in der Nachbeobachtungsphase an. Im Falle eines MI führt eine invasive Strategie im Vergleich zum konservativen Procedere zu besseren Überlebensraten, jedoch auf Kosten eines höheren Risikos für Blutungen bei älteren Patienten. Im vorliegenden Artikel wird eine Übersicht zur Diagnose des MI beim älteren Patienten gegeben.
