14. Antepartuale Überwachung

Zusammenfassung

Die CTG-Registrierung als das am häufigsten antepartal eingesetzte fetale Überwachungsverfahren hat die in die Methode gesetzten Erwartungen nicht erfüllt. Selbst bei konsequentem Einsatz in Hochrisikokollektiven konnten weder die perinatale Morbidität noch die Mortalität signifikant gesenkt werden.
Der Wehenbelastungstest kann ebenso wie der Non-Stress-Test bei unauffälligem Testresultat sehr viel zuverlässiger eine Gefährdung des Kindes ausschließen als bei auffälligem Ergebnis eine fetale Gefährdung prognostizieren.
Die beste Reproduzierbarkeit bieten dopplersonographische Messungen von arteriellen Blutströmungsgeschwindigkeiten. Allerdings ist die Effizienz dieser Diagnostik wesentlich von der vorherigen Erkennung bzw. Präselektion des Risikokollektivs abhängig. Durch entsprechenden Einsatz dopplersonographischer Untersuchungen in den Hochrisikokollektiven und Anwendung eines geeigneten Behandlungskonzeptes lässt sich die perinatale Mortalität um ca. 30 % senken.
