20. Vergleich der laparoendoskopischen mit den offenen, anterioren Techniken

Zusammenfassung

Im Lauf des vorigen Jahrhunderts wurden viele neue Techniken entwickelt, um eine Leistenhernie zu versorgen. Netzbasierte Techniken haben wegen einer niedrigeren Rezidivrate im Vergleich zu Nahttechniken mehr und mehr an Bedeutung gewonnen und werden als Goldstandard angesehen. Das zugrunde liegende Prinzip der netzbasierten Reparation ist die Verstärkung der Hinterwand des Leistenkanals. Derzeit ist Polypropylen in Netzform das meistverbreitete Material, um dies zu erreichen. Das Material kann von vorne mit einem offenen Zugang wie bei der Lichtenstein-Operation oder von hinten wie bei den laparoendoskopischen Techniken platziert werden. Reine Nahtverfahren wie die Shouldice-Technik werden vor allem in bestimmten spezialisierten Zentren weiterhin angewendet. In diesem Kapitel werden die Ergebnisse der offenen Techniken der Leistenhernienreparation kritisch denjenigen der laparoendoskopischen Techniken gegenübergestellt.
