Periprothetische Frakturen – Diagnostik, Klassifikation und Therapie

Zusammenfassung

Hintergrund

Aufgrund der durch den demografischen Wandel bedingten Zunahme endoprothetischer Eingriffe bei gesteigertem Aktivitätsanspruch unter seneszenziell bedingten Begleiterkrankungen ist eine stetige Zunahme periprothetischer Frakturen zu beobachten.

Fragestellung

Inzidenz und Ursachen von periprothetischen Frakturen, Darstellung von Diagnostik und über allgemein akzeptierte Klassifikationen Ableitung einer Therapieempfehlung.

Material und Methode

Literaturrecherche von Grundlagenarbeiten und Expertenempfehlungen sowie Analyse des eigenen Patientenguts.

Fazit

Bei wachsender Zahl implantierter Endoprothesen und steigendem Patientenalter ist eine Zunahme periprothetischer Frakturen zu erwarten. Die Therapie periprothetischer Frakturen ist komplex und erfordert sowohl eine exakte Analyse der Frakturlokalisation und -morphologie als auch, die Frage nach Lockerung des einliegenden Materials zu beantworten. Auf dieser Basis kann als essenzieller Schritt die korrekte Versorgung gewählt und in einem spezialisierten Zentrum geplant werden. Mitunter muss bei geriatrischen Patienten mit begleitender Komorbidität ein individuelles Therapiekonzept erstellt werden.
