35. Pathologische Geburt und vaginaloperative Entbindung

Zusammenfassung

Prophylaxe, Diagnose und Therapie von Störungen des Geburtsverlaufes (Dystokie) gehören zu den zentralen geburtsmedizinischen Aufgaben. Das Kapitel gliedert sich nach pathogenetischen wie auch praktisch wichtigen Problemstellungen. Die Störungen des Geburtsverlaufes gruppieren sich um die Faktoren ineffiziente Wehentätigkeit, Anomalie der Geburtswege und Besonderheiten des Kindes.
Eine instrumentelle Entbindung ist zur Beseitigung einer akuten fetalen Bedrohung, eines Geburtsstillstandes oder aus mütterlicher Indikation in der Austreibungsperiode indiziert. Voraussetzung für eine erfolgreiche Durchführung ist die exakte Bestimmung von Haltung, Einstellung und Höhenstand des kindlichen Kopfes. Die zusätzliche sonographische Diagnostik erhöht die Zuverlässigkeit. Die Vakuumextraktion hat die Zangenentbindung weitgehend verdrängt. Die instrumentelle Entbindung bedingt gegenüber der Sectio eine geringere Morbidität der Mutter und führt nicht zu einer zusätzlichen fetalen Risikoerhöhung.
