Zusammenfassung

Synkopen werden in die 3 Gruppen vasovagal/neuronal vermittelt, orthostatisch-hypotensiv und kardial/kardiovaskulär eingeteilt. Ist keine klare Zuordnung möglich, sollte eine „Synkope unklarer Genese“ diagnostiziert werden. Allen Formen der Synkope liegt die plötzliche Unterbrechung des zerebralen Blutflusses von ca. 5–8 s bzw. die vorübergehende Senkung des systolischen Blutdruckwerts unter 60 mm Hg zugrunde. Unterschiedliche Begrifflichkeiten erschweren es, verbindliche diagnostische und therapeutische Empfehlungen abzuleiten. Im Folgenden werden diagnostische Schritte bei synkopaler Bewusstlosigkeit und mögliche Abklärungsalgorithmen vorgestellt. Der Schlüssel liegt in der zielgerichteten, ausführlichen Anamnese unter Berücksichtigung von Komorbiditäten. Bestimmung der Vitalparameter, Durchführung eines Orthostasetests und Untersuchung weniger Laborparameter erlauben in der Zusammenschau die Risikoabschätzung und Weichenstellung für die gezielte Betreuung von Patienten mit Synkope.
