38. Mehrlingsschwangerschaft und Mehrlingsgeburte

Zusammenfassung

Mehrlingsgeburten stellten aufgrund der assistierten Reproduktion und des steigenden Lebensalters der Gebärenden einen wachsenden Anteil aller Geburten dar, jetzt scheint sich erstmals ein rückläufiger Trend abzuzeichnen.
Essenziell ist die Unterscheidung von mono- und dichorialen Zwillingsschwangerschaften. Monoamniale und siamesische Zwillinge, die TRAP-Sequenz („twin reversed arterial perfusion“), die extreme Wachstumsdiskrepanz, die „twin anemia polycythemia sequence“ (TAPS) und das fetofetale Transfusionssyndrom bei monochorialen Zwillingen sind Konstellationen, die einzigartig bei Mehrlingsschwangerschaften auftreten.
Die perinatale Mortalität wird bei Zwillingen 3–7× höher eingestuft als bei Einlingen und kommt durch das häufigere Auftreten von Frühgeburten, Wachstumsrestriktionen, Fehlbildungen sowie durch zwillingsspezifische Komplikationen bei monochorialen Zwillingen zustande.
