23. Mehrlingsschwangerschaft und -geburt

Zusammenfassung

Mehrlingsschwangerschaften stellen für den betreuenden Frauenarzt und für das geburtshilfliche Team in der Klinik eine große Herausforderung dar. Eine detaillierte Schwangerschaftsvorsorge und eingehende Kenntnisse in der physiologischen und pathophysiologischen Entwicklung von Mehrlingen sowie der maternalen Adaptation sind wichtige Voraussetzungen für einen optimalen Schwangerschaftsausgang. Zu unterscheiden ist zwischen dichorialen und monochorialen Mehrlingen. Letztere weisen ein 4-5-fach höheres Risikoprofil auf, die häufigste Komplikation ist das Auftreten eines fetofetalen Transfusionssyndroms. Mehrlinge sollten grundsätzlich in Perinatalzentren mit geburtshilflicher Expertise und Vorhaltung eines neonatologischen und anästhesiologischen Teams entbunden werden. Eine detaillierte evidenzbasierte vorgeburtliche Aufklärung über den Geburtsmodus inkl. dessen Erfolgschancen und mögliche Komplikationen ist empfehlenswert. Grundsätzlich liegt die Entscheidung über den Geburtsmodus bei den Eltern.
