Zusammenfassung

Jede außerhalb des Cavum uteri lokalisierte Schwangerschaft ist eine Extrauteringravidität (EUG), die in >90 % der Fälle im Bereich der Tube stattfindet (Tubargravidität, Tubaria). Die Inzidenz beträgt etwa 1–2 % angegeben.

Das klinische Bild erstreckt sich von der völlig asymptomatischen Patientin über Unterbauchschmerzen mit vaginalen Schmierblutungen und einem akutem Abdomen bis hin zum hämorrhagischen Schock nach Tubarruptur. Die Vaginosonographie bildet die Grundlage der Diagnostik und wird bei nicht gesichertem Befund durch den quantitativer Serum β-HCG-Verlauf ergänzt.

Neben der operativen Therapie – laparoskopische Salpingotomie und Salpingektomie – haben sich konservative Therapiealternativen wie die systemische Gabe von Methotrexat etabliert. Ein rein abwartendes Verhalten, bei dem auf eine eventuelle Spontanresorption gewartet wird, gewinnt zunehmend an Bedeutung, zeigen doch Studien eine Erfolgsquote von 30 %.
