24. Thromboembolische Komplikationen in Schwangerschaft und Wochenbett

Zusammenfassung

Thromboembolische Erkrankungen sind in den Industrieländern eine der häufigsten Ursachen mütterlicher Mortalität. Tiefe Venenthrombosen oder Lungenembolien treten in der Schwangerschaft etwa 4–6×, im Wochenbett 20–30× häufiger auf als bei nichtschwangeren Frauen. Das Risiko steigt bereits im 1. Trimenon und bleibt im weiteren Verlauf unverändert. Immobilisation, Trauma, insbesondere operative Traumata, Dehydratation und Infektionen stellen zusätzliche Risikokonstellationen dar.
Das erhöhte Risiko thromboembolischer Komplikationen in Schwangerschaft und Wochenbett rechtfertigen die intensiven Bemühungen, von einer Thrombose bedrohte Schwangere rechtzeitig zu erkennen und prophylaktisch zu behandeln. Mit den niedermolekularen Heparinen hat sich eine Therapieoption durchgesetzt, die bei hoher Effektivität ein günstigeres Nebenwirkungsprofil aufweist. Damit haben sich für viele Frauen Möglichkeiten eröffnet, trotz individuell erhöhter Risikokonstellation eine Schwangerschaft auszutragen.
