Zur Epidemiologie des plötzlichen Herztodes

Zusammenfassung

Der plötzliche Herztod (PHT) ist ein natürlicher und unerwarteter Tod aus kardialer Ursache, der innerhalb von 1 h nach Symptombeginn eintritt und durch eine plötzliche Bewusstlosigkeit angekündigt wird. Die Inzidenz des PHT wird auf 0,1–0,2%/Jahr der Gesamtbevölkerung geschätzt. Sie steigt jenseits des 60. Lebensjahres deutlich an. Der Anteil des plötzlichen Herztodes an allen Todesfällen macht etwa 10–20% aus. Auslöser ist meist eine tachykarde Herzrhythmusstörung. Bedroht sind vor allem Personen mit akutem Myokardinfarkt. Weiterhin sind Patienten mit stark eingeschränkter linksventrikulärer Pumpfunktion auf dem Boden einer chronischen, strukturellen Herzerkrankung gefährdet. Weitere Indikatoren für einen drohenden PHT können durch nichtinvasive, elektrophysiologische Analysen aufgedeckt werden.
