5. Indikationen zur chirurgischen Therapie von Leistenhernien

Zusammenfassung

Die Entscheidung zur Sanierung einer Leistenhernie wird durch verschiedene Faktoren beeinflusst wie Ausmaß der Behinderung, subjektive Symptomatik, Größe, Aktivität des Patienten, Information durch medizinisches Personal oder soziale Medien. Die verbreitete Meinung einer drohenden Inkarzeration oder Strangulation führt zur Verunsicherung der Betroffenen. Da die Leistenbrüche des Älteren häufig asymptomatisch oder oligosymptomatisch sind, wird die Entscheidung immer wieder über Jahre hinausgeschoben. Die Watchfull-waiting-Strategie (konservative „Behandlung“ asymptomatischer oder minimal symptomatischer Hernien), um den Eingriff zu Lebzeiten vermeiden zu können, hat versagt. 2011 berichtete Chung (Arbeitsgruppe Fitzgibbons), dass die Konversionsrate der „observierten“ (nicht operierten) Gruppe 72 % nach 7,5 Jahren betrug. Diese Tatsache disqualifizierte die abwartende Strategie. Die Indikationsbreite zu einer laparoendoskopischen Hernienoperation ist über die Jahre durch zunehmende Erfahrung und Kompetenz stets gewachsen. Heute lässt sich die Indikation von einer einfachen einseitigen Hernie über die Rezidivhernie bis zu komplexen Hernien nur auf der Basis der notwendigen Expertise stellen.
