Fragilitätsfraktur beim geriatrischen Patienten: Präoperative Abklärung und Optimierung

Zusammenfassung

Fragilitätsfrakturen gehen mit hoher Morbidität und Mortalität einher. Eine interdisziplinäre Zusammenarbeit und ein individuelles, patientenzentriertes Vorgehen sind entscheidend für eine optimierte präoperative Phase und verbesserte perioperative Sicherheit. Präoperative Aufgaben der Unfallchirurgie sind im ersten Schritt die Identifikation als Fragilitätsfraktur und des geriatrischen Handlungsbedarfs. Das orthogeriatrische Co-Management (OCM) dient der Identifikation der medizinischen, funktionellen und sozialen Bedürfnisse des Patienten. In der präoperativen Phase stehen akut behandlungsbedürftige Erkrankungen, die den weiteren Behandlungsverlauf negativ beeinflussen, und Delirprophylaxe im Vordergrund.
