Akutes Koronarsyndrom

Zusammenfassung

In aktuellen Leitlinien wird beim akuten Koronarsyndrom (AKS) zwischen Fällen ohne ST-Strecken-Hebung und solchen Fällen mit ST-Strecken-Hebung im EKG unterschieden. Patienten mit AKS-typischen Beschwerden ohne ST-Strecken-Hebung, aber mit erhöhten Werten für Kreatinkinase bzw. CK-MB und vor allem mit erhöhten Troponinwerten (T oder I) haben eine sehr ungünstige Prognose. Das AKS ist die häufigste Einsatzindikation für den Notarzteinsatz, daher muss ein 12-Kanal-EKG-Gerät vorgehalten und ein EKG unverzüglich bei Verdacht auf AKS zu abgeleitet werden. Die prästationäre Lyse ist eine wichtige Option, insbesondere bei Symptomdauer unter 3 h. Daher wird die Forderung nach der Finanzierung der prähospitalen Thrombolyse unterstützt.
