Zusammenfassung

Feten mit restringiertem Wachstumspotenzial können dieses nicht vollständig ausschöpfen. Bei dieser Diagnosestellung spielt die Dopplersonographie, v. a. bei früher („early-onset“) fetaler Wachstumsrestriktion bis zu 34 SSW, eine wichtige Rolle. Mit fortgeschrittenem Schwangerschaftsalter zeigen die meisten SGA-Feten („small for gestational age“) keine Auffälligkeiten im Umbilikalarteriendoppler. Neuere Studien zeigen, dass auch diese „late-onset“ SGA-Feten (>34 SSW) signifikant häufiger Geburtsprobleme, eine suboptimale neurologische Entwicklung und höhere kardiovaskuläre Risiken haben. Im Gegensatz dazu steht die frühe „early-onset“ FGR, die mit den typischen Dopplerveränderungen einhergeht, mit hoher Morbidität und Mortalität vergesellschaftet ist und v. a. vom Schwangerschaftsalter bei Diagnose abhängt.

Die große Herausforderung der frühen FGR ist die Optimierung des Entbindungszeitpunktes, die Schwierigkeit der späten FGR ist bereits die diagnostische Erfassung.
