Zusammenfassung

Anamnese und klinischer Befund | Ein Patient mit stärksten Schulterschmerzen beidseits präsentierte sich in der Notaufnahme.

Untersuchungen | Im initialen Labor zeigten sich deutlich erhöhte Transaminasen.

Diagnose | Serologisch und mittels PCR konnte eine Hepatitis festgestellt werden. Die Neurologische Abklärung erbrachte eine Neuralgische Amyotrophie.

Therapie und Verlauf | Die Schulterschmerzen sistierten spontan nach verschwinden der HEV-Virämie.

Folgerung | Patienten mit Schulterschmerzen unklarer Genese und erhöhten Transaminasen sollten auf Hepatitis E getestet werden.

Abstract

History and initial findings | A patient with bilateral pain in his shoulders is presented at the emergency room.

Investigations | There were strongly increased transaminases.

Diagnosis | Serology and PCR led to the diagnosis of hepatitis E. Neurological examination revealed a neuralgic amyotrophy

Treatment and course | The pain in the shoulders disappeared spontaneously after vanishing of HEV viremia.

Conclusion | Patients with shoulder pain of unknown origin and elevated transaminases should be tested for hepatitis E.
