Zusammenfassung

Soteria bezeichnet einen milieutherapeutischen Behandlungsansatz bei akuten Psychosen mit persönlicher Begleitung in einer wohngemeinschaftsähnlichen Umgebung und behutsamer neuroleptischer Medikation. Auch nach vier Jahrzehnten überwiegend positiver klinischer und empirischer Erfahrung wird die Soteria noch immer primär aus moralischen und humanistischen Gründen als wünschenswert erachtet. Ergänzend zu dieser tendenziell einseitigen Rezeption wollen wir aufzeigen, dass sich die Anwendung des Soteria-Konzepts auch direkt, also im Rückgriff auf störungsimmanente Faktoren und spezifische Charakteristika der Schizophrenie selbst begründen lässt. Hierfür gehen wir von einem dezidiert phänomenologischen Verständnis schizophrener Störungen aus, mit dessen Hilfe die Notwendigkeit lebensweltlich eingebetteter Behandlungsformen zur Förderung alltäglicher und sozialer Fertigkeiten begründet und klinische Konsequenzen so dargestellt werden sollen, wie sie in der neu gegründeten Soteria-Station am Zentrum für Psychiatrie Reichenau bereits angewandt werden.

Abstract

Soteria is an alternative approach to the treatment of schizophrenia that focuses on human interaction (“being-with”) within a residential community and the cautious use of neuroleptic medication. Despite forty years of promising clinical and empirical evidence, the need for facilities based on the Soteria paradigm is still largely grounded on humanitarian and “moral” reasons. In addition to this, we will try to demonstrate that Soteria might also prove to be a promising approach based on scientific reasons immanent to the disorder. From a phenomenological informed conception of schizophrenia we will explicate clinical implications, specifically the need for a holistic treatment in real life settings, and demonstrate how these conclusions can most adequately be implemented within Soteria.
