AIM: The study investigates how a psychoanalytic institution reaches its aims by identifying different factors like diagnosis, sociodemographic characteristics of the patient, motivation of the psychoanalyst, and their impact on the indication-process. MATERIAL AND METHODS: 38 patients diagnosed as having severe personality disorders and treated in a psychoanalytic outpatient clinic were compared with 49 patients who were referred for psychoanalytic treatment to a private practice. RESULTS: Although the groups were similar regarding the severity of their problems, more comorbidity and a more stable professional life were found in the group referred to private practice. Patients who tend to distort their social life in order to maintain a fragile psychic balance are more likely to get institutional treatment. The psychoanalytic team-conference and discussion of unconscious object-relationship pattern had great impact on the process of giving correct indications. CONCLUSION: The indication for psychoanalytic treatment does not correlate with personality disorder diagnosis or with the motivation of the therapist. Nevertheless, more differentiated, in clinical routine practicable instruments measuring reflective functioning, or the quality of object relationship have to be developed in order to give an adequate indication for psychoanalytic treatment.
Zusammenfassung
ZIEL: Die Studie untersucht die Frage, wie eine Institution der Aufgabe, direkt durch die Krankenkasse vollständig finanzierte Psychoanalyse für Patienten mit schweren Persönlichkeitsstörungen zukommen zu lassen, gerecht wird. Der Einfluss von Faktoren wie der Diagnose, der soziodemographischen Merkmale der Patienten und der Motiviertheit des Therapeuten auf die Indikationsstellung wurde evaluiert. MATERIAL UND METHODIK: 38 Patienten mit schweren Persönlichkeitsstörungen, die eine institutionelle psychoanalytische Behandlung erhielten, wurden mit 49 Patienten verglichen, die zu niedergelassenen Psychoanalytikern weiter verwiesen wurden (Gruppenvergleich). ERGEBNISSE: Bei der Untersuchung der Inanspruchnahme psychoanalytisch psychotherapeutischer Hilfe bei Persönlichkeitsstörungen in der Institution und der Gegenüberstellung der Daten mit denen einer Vergleichsgruppe zeigten sich bei durchschnittlich gleicher Fallschwere Unterschiede im soziodemographischen Merkmal der Berufstätigkeit und im Bereich in der Komorbidität. Die Therapieempfehlungen sind unabhängig von der Persönlichkeitsstörungsdiagnose des Patienten und von der Motivation des Therapeuten. Die wesentliche Entscheidung für die Indikationsstellung wird in der Ambulanzkonferenz getroffen, indem die Objektbeziehungsqualität und Mentalisierungsfähigkeit reflektiert werden. KONKLUSION: Diejenigen Patienten, die unbewusst ihre "Realität leiden lassen", die vor allem ihre Lebensumstände dazu verwenden, ihr prekäres psychisches Gleichgewicht aufrecht zu halten, erhalten eine institutionelle Psychoanalyse. Für die empirische Darstellung der Evaluation der Objektbeziehungsqualität und Mentalisierungsfähigkeit, die wesentliche Implikationen für die Indikationsstellung zeigten, sollten weitere diagnostische Instrumente erprobt und entwickelt werden.
