Zusammenfassung
Amiodaron gilt als das zurzeit effektivste Antiarrhythmikum in der Behandlung von Patienten mit supraventrikulären und ventrikulären tachykarden Herzrhythmusstörungen. Der antiarrhythmische Effekt von Amiodaron beruht im Wesentlichen auf einer Verlängerung der kardialen Repolarisation. Amiodaron wird daher den Antiarrhythmikaeigenschaften der Klasse III nach Vaughan-Williams zugerechnet. Im Vergleich zu anderen Antiarrhythmika hat Amiodaron eine niedrige Proarrhythmierate (Häufigkeit ca. 1%), die durch zusätzlich bestehende vielfältige Kanal- und Wirkeffekte von Amiodaron erklärt werden. Amiodaron ist das Antiarrhythmikum bei therapierefraktärem Kammerflimmern, wenn DC-Kardioversionen nicht erfolgreich sind. Es kann auch bei Patienten mit supraventrikulären Tachyarrhythmien akut und im Langzeitverlauf eingesetzt werden, besonders bei Patienten mit Vorhofflimmern und schlechter linksventrikulärer Pumpfunktion. Der Langzeitverlauf von Amiodaron ist allerdings durch zum Teil häufige, zum Teil schwere Nebenwirkungen belastet. Deshalb sind strenge Indikationsstellung und kontinuierliche Kontrolluntersuchungen unerlässlich.
Abstract
Amiodarone (Amio) is currently regarded as the most effective antiarrhythmic drug available for the treatment of patients (pts) with supraventricular and ventricular tachyarrhythmias. The most relevant antiarrhythmic effect of amiodarone is due to its prolongation of cardiac repolarization. Amio has been classified as a class III drug after the Vaughan Williams classification. However, its low rate of proarrhythmic complications (incidence ca. 1%) as compared to other class III drugs is at least in part explained by its multiple actions on different ionic channels and ion currents. Amio is the drug of choice in pts with refractory ventricular fibrillation even after unsuccessful DC defibrillation. Amio is effective acutely and during long-term follow-up in pts with supraventricular tachyarrhythmias, especially in pts with atrial fibrillation and low left ventricular function. Despite excellent clinical results, there are frequent and severe side effects on Amio. Therefore, strict indications for this treatment and continuous check-ups are necessary.
