Zusammenfassung
Notaufnahmen haben die Aufgabe der primären Stabilisierung und der Initialdiagnostik, die für eine optimale Weiterversorgung der Patienten unabdingbare Voraussetzung ist. Damit entwickeln sich die Notaufnahmen von einem Ort, der primär der Verteilung der Patienten in die verschiedenen Fachabteilungen diente, zu einer gut definierten Funktionseinheit mit einem eigenständigen fachlichen Profil. Besonderes Augenmerk verdienen dabei intensivmedizinische Kenntnisse, die es ermöglichen, den Verlauf einer kritischen Krankheit zu antizipieren, rasch zu intervenieren und so längerfristige Intensivaufenthalte zu reduzieren. Die Intensivkompetenz für Notfallaufnahmen ist zu fordern, weil damit die Prognose der Notfallpatienten verbessert wird, weil dies ökonomisch sinnvoll ist und weil damit Intensiv- und Normalstationsbetten für Patienten, die diese wirklich benötigen, gesichert werden.
Abstract
Emergency departments are responsible for primary stabilization and initial diagnostic procedures to ensure the best possible ongoing patient care. They are thus evolving from a service primarily used to distribute patients to the various specialist departments into a well-defined functional unit with its own profile of expertise. Particular attention should be paid to intensive care skills, which allow the course of a critical illness to be anticipated and rapid action to be taken, hence reducing long stays in intensive care. Intensive care skills are necessary for emergency departments because they improve the prognosis for emergency patients, because they make economic sense and because they ensure that beds in intensive care units and on normal wards are reserved for the patients that really need them.
