Abstract
Mild therapeutic hypothermia after cardiac arrest reduces mortality and neurologic outcome. Recently, its use has been recommended by the International Liaison Committee on Resuscitation (ILCOR). Several methods and cooling devices have been reported so far. We describe an extracorporeal cooling device using cold water circulating in two mats to achieve and maintain a core temperature of 32–34 °C. In this study the target temperature was reached in all patients (n = 21). By lowering the temperature in the circulating water system to 5 °C, we managed to obtain the target temperature in 120 min. The presented cooling system was very effective in daily clinical routine. Establishing such a simple cooling devicemay facilitate the implementation of therapeutic hypothermia according to the ILCOR-guidelines.
Zusammenfassung
Die milde therapeutische Hypothermie nach Reanimation verbessert die Mortalität der Patienten und vermindert das Ausmaß neurologischer Schäden. Seit 2003 wird dieses therapeutische Verfahren von der International Liaison Committee on Resuscitation (ILCOR) empfohlen. Es stehen verschiedene Verfahren zur effektiven Absenkung der Körpertemperatur zur Verfügung. Wir berichten erstmals über den Einsatz eines neuen externen Kühlaggregates, bei welchem die Abkühlung auf 32–34 °C über ein geschlossenes Kühlwassersystem und zwei Kühlmatten erfolgt. In vorliegender Studie wurde bei allen 21 Patienten mit diesem System die Zieltemperatur erreicht. Die hierfür benötigte Zeit konnte durch Absenken der Wassertemperatur im Kühlkreislauf auf 5 °C auf im Mittel 120 min verkürzt werden. Damit steht für die klinische Routineanwendung ein technisch einfaches, kostengünstiges und nicht-invasives System zur Durchführung einer effektiven therapeutischen Hypothermie zur Verfügung. Die bisher zögerlich umgesetzten ILCOREmpfehlungen könnten durch dieses neue Hypothermieaggregat eine größere Akzeptanz erfahren.
