Summary
The therapy of strokes within a clinical center affords an advantage regarding patient survival and functional outcome. It is the responsibility of the rescue service to diagnose a suspected stroke, stabilize impaired vital functions and to transfer the patient to a suitable clinic as quickly as possible. To assess how the preclinical management of time and treatment can be optimized, we retrospectively analyzed all cases of the year 2003 in which a doctor responded to an emergency call and the diagnosis was a suspected transient ischemic attack (TIA), stroke, or intracerebral bleeding (ICB). Most of the time, when the emergency doctor was notified, the suspected stroke patient was given clinical treatment rapidly. In comparison with other clinical centers, we noticed substantial improvement for certain aspects of the preclinical treatment of stroke patients. Nevertheless, also at our clinic there is a considerable need for treatment optimization. We propose a standard operating procedure (SOP) for the preclinical care of stroke patients which should optimize, in particular, the blood pressure management, the responses to impaired protective reflexes and to hyperthermia, the quality of documentation as well as the time management.
Zusammenfassung
Die Therapie von Schlaganfällen in Zentren (Stroke unit, Lysetherapie, operative Versorgung) bietet Patienten mit apoplektischem Insult einen Vorteil hinsichtlich Überleben und funktionellem Outcome. Aufgabe des Rettungsdienstes ist es, die Verdachtsdiagnose „Schlaganfall“ zu stellen, gestörte Vitalfunktionen zu stabilisieren sowie die Patienten zügig einer geeigneten Klinik zuzuweisen. Um Anhaltspunkte für eine Optimierung des präklinischen Zeit- und Behandlungsmanagements zu gewinnen, werteten wir für das Jahr 2003 retrospektiv alle Notarzteinsätze mit der Verdachtsdiagnose „TIA/Insult/ICB“ aus. Bei Alarmierung des Notarztes wurden Patienten mit vermutetem Schlaganfall zumeist zügig einer klinischen Versorgung zugeführt. Im Vergleich zu anderen Zentren waren zwar substantielle punktuelle Verbesserungen im präklinischen Schlaganfallmanagement zu beobachten, dennoch besteht auch an unserem Standort noch erheblicher Optimierungsbedarf. Wir stellen einen standardisierten Behandlungsablauf (SOP) für die prähospitale Versorgung von Patienten mit Schlaganfall vor, der insbesondere das Blutdruckmanagement, das Vorgehen bei gestörten Schutzreflexen und Hyperthermie, die Qualität der Dokumentation, sowie das Zeitmanagement optimieren soll.
