Summary

The primary in-hospital assessment of patients suffering from acute impairment of consciousness in an emergency admitting hospital unit requires a high level of differential diagnostic skills and necessitates immediate and concise decisions with respect to emergency diagnostic and treatment procedures. The aim of this consensus paper is to delineate the relevant diagnostic neurological and neurosurgical procedures in patients with impaired consciousness and to elucidate the decision-making pathway for extended diagnostic and treatment procedures. The diagnostics and documentation of impaired consciousness should be performed clinically with documentation using an accepted coma scale. The relevant differential diagnostic categories should be considered. The immediately necessary laboratory tests and technical examinations should be limited to the most important items and need to be performed in a timely fashion. In a next step, the primary treatment measures are listed. Finally, the legal and ethical questions of treatment of patients without the ability to provide informed consent are discussed in the light of patient manifestos in emergency situations. The consensus paper proposes a management guideline for the primary in-hospital emergency care of patients with impaired consciousness.

Zusammenfassung

Die Erstversorgung komatöser Patienten in einer Notaufnahmeeinheit stellt hohe Anforderungen an differentialdiagnostische Fähigkeiten der beteiligten Ärzte und macht rasche Entscheidungen in Hinblick auf weiterführende Diagnostik und Akuttherapie notwendig. Ziel des Konsensuspapiers ist es, die wesentlichen klinisch-neurologische und neurochirurgischen Differentialdiagnostik bei bewusstseinsgetrübten und komatösen Patienten darzustellen und die Entscheidungswege für die initiale apparative Diagnostik sowie für die Akuttherapie aufzuzeigen. Die Diagnose und Dokumentation der Bewusstseinsstörung und die primäre klinische Diagnostik sind als erste Schritte durchzuführen. Die wichtigsten differentialdiagnostischen Überlegungen zu Ursachenklärung werden aufgezeigt. Die vordringlich durchzuführende laborchemische und apparative Diagnostik wird erläutert. In einem weiteren Abschnitt werden erste therapeutische Maßnahmen aufgelistet. Schließlich werden Fragen der Versorgung von nichteinwilligungsfähigen Patienten und der Stellenwert von Patientenverfügungen in der Notfallsituation erörtert. Das Konsensuspapier stellt einen Handlungskorridor für die Erstversorgung von akut bewusstseinsgestörten Patienten dar.
