Letale Staphylococcus aureus-Sepsis nach intramuskulärer Injektion

Abstract

A 67-year-old man showing symptoms of septic shock died 24 h after admission to hospital, four days after receiving intramuscular injection of an antirheumatic to the right gluteal region for treatment of acute lumbago. The reason for septicemia was clinically unknown. Autopsy showed non-specific changes caused by shock. A circumscribed abscess was found in the right gluteal region where the intramuscular injection took place: histological examination showed small clusters of bacteria (Staphylococcus aureus by bacteriological diagnosis). The intramuscular injection was confirmed as the origin of the Staphylococcus aureus sepsis.

Zusammenfassung

Berichtet wird über einen 67jährigen Mann, der notfallmäßig unter der Symptomatik eines septischen Schocks stationär aufgenommen wurde und 24 Stunden nach Aufnahme, ohne daß klinisch der Ausgangspunkt der Sepsis verifiziert werden konnte, verstarb. Vorausgegangen war die intramuskuläre Injektion eines nichtsteroidalen Antirheumatikums in die rechte Glutaealregion wegen akutem Lumbago vier Tage zuvor. Bei der Obduktion fanden sich unspezifische Schockzeichen. Im Fettgewebe der rechten Glutaealregion, in der die intramuskuläre Injektion erfolgte, fand sich ein Abszeß mit histologischem Nachweis von Bakterienrasen (kulturell Staphylococcus aureus). Die intramuskuläre Injektion ist als unmittelbarer Ausgangspunkt der letal verlaufenen Staphylococcus aureus-Sepsis und damit als todesursächlich anzusehen.
