Zusammenfassung

Die Arbeit gibt eine Übersicht zur gegenwärtigen Klassifikation, Beschreibung und Behandlung derjeniger chronischer Schmerzen, bei denen psychosozialen Faktoren eine (mit)ursächliche Rolle zugesprochen wird. Sie stützt sich methodisch auf die bei der Erstellung der „Leitlinien Somatoforme Störungen“ und bei einer entsprechenden Metaanalyse gewonnenen Erkenntnisse. Die klassifikatorischen Probleme v. a. in der Abgrenzung somatoformer von anderen Schmerzstörungen werden dargestellt; es folgt eine Konkretisierung relevanter psychosozialer Beschreibungsdimensionen anhand empirischer Befunde: Art der Schmerzbeschreibung, Zahl anderer nicht ausreichend organisch erklärter Körperbeschwerden, Angst und Depressivität, Ursachenüberzeugung und Krankheitsverhalten, Kindheitsbelastung. Die auf dieser diagnostischen Grundlage modifizierte störungsorientierte Psychotherapie bei (somatoformen) chronischen Schmerzen wird skizziert; abschließend erfolgt eine Diskussion des Bezugs dieser ätiologisch-kurativen Herangehensweise der Psychosomatik und Psychiatrie zur bewältigungsorientierten Perspektive der Medizinpsychologie.

Abstract

An overview is given on the current classification, description and treatment of chronic pain with causally relevant psychological factors. It is based on the “practice guidelines on somatoform disorders” and on a thematically related meta-analysis. The classificatory problems, especially of the demarcation of somatoform and other chronic pain, are presented. Additional descriptive dimensions of the relevant psychosocial factors are: pain description, other organically unexplained pain- and non-pain-symptoms, anxiety and depression, disease conviction and illness behaviour, personality and childhood abuse. A modified psychotherapy for (somatoform) chronic pain is outlined. Finally, this aetiologically oriented psychosomatic-psychiatric approach is compared to psychological coping models for chronic pain.
