Amplitude-duration relation for direct ventricular defibrillation with rectangular current pulses

Abstract

In this study it is demonstrated that, for defibrillation with a rectangular pulse, the relationship between threshold current strength and pulse width can be approximated by a hyperbola. Also the threshold charge and energy required for effective defibrillation are presented as functions of the pulse duration. From these three curves it can be seen that, for a rectangular pulse, the current strength is minimal for a duration between 20 and 40 ms, the charge, for a ‘zero’ duration and the energy for a duration of around 4 ms. Myocardial performance is generally assumed to be, minimally disturbed after a defibrillation shock if the applied current, charge and energy of one pulse are all minimal. Then it can be concluded that, for a rectangular pulse, no duration can be advised for defibrillation with the least myocardial disturbance after the shock, since current, charge, and energy have their minimums at different, pulse durations.

La présente étude démontre que pour la défibrillation avec des impulsions, rectangulaires, on peut assimiler par approximation à une hyperbole le rapport entre le courant de seuil et la largeur d'impulsion. De même, la charge de seuil et l'énergie requises pour une défibrillation efficace sont présentées en fonction de la durée d'impulsion. Il ressort de ces trois courbes que, pour une impulsion rectangulaire, l'intensité du courant est minimale pour une durée comprise entre 20 et 40 ms, la charge pour une durée “nulle” et l'énergie pour une durée d'environ 4 ms.

La performance myocardiale est généralement acceptée comme faisant l'object d'une perturbation minimale après un choc de défibrillation si le courant appliqué, la charge et l'énergie d'une impulsion sont tous minimum. On peut alors conclure que pour une impulsion rectangulaire aucune durée ne peut être conseillée pour la défibrillation avec la perturbation myocardiale minimum après le choc, étant donné que le courant, la charge et l'énergie ont leur, minimum à différentes durées d'impulsions.

Zusammenfassung

In dieser Untersuchung wird gezeigt, daß man das Verhaltnis zwischen Schwellenstromstärke und Impuls-breite bei der Defibrillation mit Rechteckimpuls durch eine Hyperbel annähernd bestimmen kann. Ferner werden Schwellenladung und-energie, die zur wirskamen Defibrillation erforderlich sind, als eine Funktion der Impulsdauer dargestellt. Aus diesen drei Kurven kann entnommen werden, daß die Stromstärke bei einem Rechteckimpuls für eine Dauer von 20 bis 40 ms, der Ladung für “Null”-Dauer und der Energie für eine Dauer von etwa 4 ms minimal ist.

Es wird allgemein angenommen, daß die Myokardleistung nach einem Defibrillationsschock minimal gestört ist, wenn der angewandte Strom, die Ladung und die Energie eines Impulses alle minimal sind. Daraus kann dann geschlossen werden, daß bei einem Rechteckimpuls für die Defibrillation mit der geringsten Myokardstörung nach dem Schock keine Dauer berücksichtigt werden kann, da Stromstärke, Ladung und Energie ihre Mindestwerte bei unterschiedlicher Impulsdauer erreichen.
