Zusammenfassung

Die Gesundheitsökonomische Evaluation von E-Health-Anwendungen stellt Gesundheitsökonomen, Ärzte und weitere Akteure im Gesundheitswesen vor besondere Herausforderungen. Klassische gesundheitsökonomische Evaluationsmethoden und medizinische Goldstandards wie Randomisiert-Kontrollierte-Studien können nicht ohne weiteres übertragen werden. Im vorliegenden Kapitel werden technische und ethische Schwächen gängiger Evaluationsmethoden im Kontext von E-Health aufgezeigt und daran Lösungsansätze abgeleitet.

    Keine vergleichende Form der gesundheitsökonomischen Analyse.

    Denkbar ist auch eine negative Lebensqualität, bei der sich der Patient auf Grund von Schmerzen oder anderen Faktoren nach dem Tod sehnt.

    Der Nutzen selbst, sowie die Einheit, in der der Nutzen gemessen werden soll, ist abhängig vom jeweiligen Kontext zu bestimmen bzw. festzulegen.

    Intangibler Nutzen wird hingegen in der Regel in gesundheitsökonomische Bewertungen einbezogen. So unter anderem in der Lebensqualität, die auch als Grundlage für die Berechnung von QALYs dient.

    Es kann unter bestimmten Umständen allerdings auch exakt zum Gegenteil kommen, dass das Krankenhaus, welches im Bereich der Digitalisierung weit vorangeschritten ist, Schwierigkeiten hat, neue Systeme in das bestehende IT-Netz einzubinden. Schnittstellenproblematiken sind weit verbreitet.

    Zum Beispiel könnte bei psychotischen Patienten beim Tragen eines elektronischen Gerätes öfter die Angst entstehen, durch das Gerät überwacht und gesteuert zu werden.

    In einigen Bereichen wie zum Beispiel den digitalen Gesundheitsanwendungen (§33a SGB V) gibt es schon umfassende Regeln zum Thema des Nutzennachweises.
