Zusammenfassung
Bei einer intraabdominellen Volumenzunahme steigt der intraabdominelle Druck, zuerst aufgrund elastischer Begrenzungsstrukturen (Zwerchfell, Bauchwand) langsam, bei fortschreitender Volumenzunahme aber exponentiell. Das hieraus resultierende akute abdominelle Kompartmentsyndrom (AKS) kann bei übersehener Diagnose und fehlender Entlastung zur Organdysfunktion bis zum Multiorganversagen führen.
Das abdominelle Kompartmentsyndrom (AKS) ist definiert als eine Kombination einer pathologischen intraabdominelle Druckerhöhung (IAD) von >20 mmHg und dem gleichzeitigem Vorliegen progressiver Organdysfunktionen.
Die Inzidenz des AKS liegt nach großen chirurgischen Eingriffen bei ca. 10 %. Auch kritisch kranke internistische Patienten können ein AKS entwickeln, welches in der Regel einen langsameren Verlauf nimmt und zunächst medikamentös (Volumenmanagement, diuretische Therapie) behandelt wird. Die Mortalität bei internistischen Patienten ist vergleichbar zum AKS des chirurgischen Patienten.
Die frühzeitige Diagnose des AKS beruht zunächst auf der Anamnese und der Erhebung von Risikofaktoren und der klinischen Symptomatik. Die Früherkennung ist kritisch, da nur durch die frühzeitige Diagnose und Therapie die Abwärtsspirale bis zum Multiorganversagen verhindert werden kann.
