Patientinnenzufriedenheit und Outcome nach Prolapsoperationen in einem ländlichen Spital
Abstract
Zusammenfassung. Untersucht wurden objektive und subjektive Langzeitergebnisse vaginaler und laparoskopischer Prolapsoperationen in einem allgemeinen gynäkologischen Lehrkrankenhaus. Es handelt sich um eine retrospektive Studie mit 269 Frauen. Das subjektive Ergebnis wurde anhand des P-QOL-Fragebogens und Befragungen beurteilt, die objektiven Ergebnisdaten wurden prä- und postoperativ nach drei bis neun Monaten beurteilt. Die Krankengeschichte wurde bezüglich Anamnese sowie intra- und postoperativen Komplikationen analysiert. Wir führten 87 laparoskopische Sakrokolpopexien, 100 Kolpofixationen nach Richter, 164 anteriore oder/und posteriore vaginale Plastiken, oft in Kombination mit anderen Eingriffen, sowie weitere Eingriffe durch. 80 % (185/206) der behandelten Frauen waren mit dem postoperativen Ergebnis zufrieden. Kenntnisse der Anamnese und der Symptome der Patientin sind sehr wichtig, um die Frauen über chirurgische Optionen und Erwartungen hinsichtlich des objektiven postoperativen Ergebnisses orientieren zu können.
Abstract. We assessed the objective and subjective long-term outcome of vaginal and abdominal prolapse surgery in a general gynecological teaching hospital. This is a retrospective study with 269 women. Subjective outcome was assessed using the P-QOL questionnaire and objective outcome data was assessed pre- and postoperatively at three to nine months. Complete chart reviews regarding medical history as well as intra- and postoperative complications were scanned. We performed 87 laparoscopic sacrocolpopexies, 100 sacrospinous fixations, 164 anterior or posterior colporrhaphies, often in combination with other procedures, and some other interventions. 80 % (185/206) of the treated women reported satisfaction with the postoperative result. Knowledge of medical history and symptoms of the patient are very important in order to advise and inform the women about surgical options and with respect to the expectations regarding objective and subjective outcome.
Résumé. Nous avons évalué les résultats objectifs et subjectifs à long terme de la chirurgie du prolapsus dans un hôpital d’enseignement gynécologique général. Il s’agit d’une étude rétrospective de 269 femmes. Le résultat subjectif a été évalué à l’aide du questionnaire P-QOL et les données objectives sur les résultats ont été évaluées avant et après l’intervention à 3–9 mois. Examen complet des dossiers médicaux concernant les antécédents médicaux, les complications intra- et postopératoire ont été analysées. Nous avons réalisé 87 sacro-colpopexies laparoscopiques, 100 fixations sacro-épineuses, 164 colporrhaphies antérieures ou postérieures souvent associées à d’autres operations, et autres interventions. 80 % (185/206) des femmes traitées ont déclaré être satisfaites du résultat postopératoire. La connaissance des antécédents médicaux et des symptômes de la patiente est très importante afin de conseiller et d’informer les femmes sur les options chirurgicales, ainsi que les attentes concernant les résultats.
