Zusammenfassung
Das anästhesiologische Vorgehen bei der elektiven Tracheotomie ist von der raschen Eröffnung des Atemweges im Notfall zu unterscheiden. Während im ersten Fall eine Narkose mit kurzwirksamen intravenösen Medikamenten bevorzugt wird, oder beim bereits beatmeten Patienten der Intensivstation die bestehende Sedierung vertieft und ergänzt wird, muss in der Situation des Notfalls die Erhaltung der Spontanatmung unter Sauerstoffinsufflation oder unter Lokalanästhesie mit nur geringer oder keiner Sedierung angestrebt werden. Die elektive Tracheotomie wird klassischerweise in Intubationsnarkose durchgeführt, um während des gesamten Ablaufs einen reflexfrei narkotisierten Patienten und einen sicheren Atemwegszugang zu garantieren. Als Alternativen zur konventionellen endotrachealen Intubationstechnik können z. B. die Larynxmaske oder das starre Tracheotomie-Endoskop erwogen werden.
