Zusammenfassung

Traditioneller Goldstandard in der Diagnostik peripherer Neuropathien ist die klinische Untersuchung, ergänzt durch die elektrophysiologischen Verfahren der Elektromyografie und Neurografie. Die spezialisierte Bildgebung peripherer Nerven mit Methoden der MR-Neurografie und Nervensonografie hat in den letzten Jahren weitreichenden Eingang in Primär- und Verlaufsdiagnostik peripherer Neuropathien gehalten. Insbesondere bei der exakten Lokalisationsdiagnostik können hochauflösende Bildgebungsmethoden ihre Stärken ausspielen, vor allem wenn Läsionsorte und -muster mit klinisch-neurophysiologischen Verfahren nicht oder nicht genau genug eingegrenzt werden können. Die MR-Neurografie zeichnet sich durch einen besonders starken Läsionskontrast bei spontanen und nichtspontanen Neuropathien und Polyneuropathien aus und eine räumlichen Auflösung, die es ermöglicht, die faszikuläre Feinstruktur peripherer Nerven abzubilden.
