Zusammenfassung
Der Einsatz des thorakalen Ultraschalls zur Diagnose des interstitiellen Syndroms ist eine der am leichtesten zu erlernenden Ultraschalltechniken. Die interstitielle Lungenbeteiligung bei Herzinsuffizienz, ARDS, Lungenfibrose und interstitiellen Lungeninfektionen weist ein ähnliches sonografisches Muster auf, das sich bei kritisch kranken und notfallmedizinischen Patienten sehr leicht mit dem Thoraxultraschall am Krankenbett erkennen lässt. Die Technik basiert auf der Erkennung von Lungengleitern und einigen vertikalen Artefakten, den B-Linien, die durch Mehrfachreflexionen des Ultraschallstrahls entstehen, der zwischen dem Alveolarinhalt und wasserreichen Strukturen gefangen ist. Die Diagnose des diffusen interstitiellen Syndroms stützt sich auf 3 grundlegende Schritte: Erkennung von B-Linien, die auf dem Bildschirm als laserartige vertikale echogene Artefakte erscheinen, die von der Pleuralinie ausgehen, sich in die Tiefe ausbreiten, ohne zum Rand des Bildschirms zu verblassen, und sich synchron mit dem Lungengleiten bewegen. Die Diagnose einer positiven Einzeluntersuchung steht, wenn die B-Linien mehrfach und dicht beieinander liegen. Die Diagnose einer positiven Untersuchung ist definiert als mindestens zwei positive Scans pro Seite. Einzelne positive Scans weisen auf das fokale interstitielle Syndrom hin, das eine andere Bedeutung hat und in der Umgebung von alveolären Konsolidierungen oder sogar in normalen Lungen sichtbar gemacht werden kann.
