Zusammenfassung

In Deutschland fand nach der Psychiatrie-Enquête eine Umstrukturierung des Hilfesystems für Menschen mit schweren psychischen Erkrankungen mit einer Verlagerung von Ressourcen vom stationären Sektor zur wohnortnahen teilstationären und ambulanten Versorgung statt. Gleichzeitig erfolgte im stationären Bereich eine Regionalisierung der Versorgung mit Reduktion der Bettenzahl psychiatrischer Fachkrankenhäuser und eine Verschiebung hin zu neueröffneten psychiatrischen Fachabteilungen an Allgemeinkrankenhäusern. Die stationäre Behandlung konnte so zunehmend gemeindenäher erfolgen. In der Folgezeit wurden und werden vor allem in den USA und England Versorgungsmodelle erprobt, die eine intensive Betreuung von Betroffenen mit schweren psychischen Erkrankungen, darunter auch der Schizophrenie, ermöglichen und die Notwendigkeit stationärer Aufnahmen begrenzen sollten. Von diesen Versorgungsmodellen stellten sich vor allem die aufsuchende gemeindepsychiatrische Behandlung im Team (Assertive Community Treatment, ACT) und gemeindepsychiatrische Teams (Community Mental Health Teams) als wirksam heraus [161, 162, 927].
