Es war kurz vor Mitternacht und Dr. Inficio seine Nachtschicht war noch jung. „Der Notarzt mit dem Zugang ist da. Kommst du?“ Dr. Inficio sah auf und nickte. „Gib mir noch 3 Sekunden, Sarah! Ich bin gleich da!“ Es war nicht sein bester Tag bzw. seine beste Nacht heute. Dr. Inficio hatte gerade von seinem Hintergrunddienst OA Dr. Volkrad einen telefonischen Einlauf erhalten, weil er übersehen hatte, bei einem orthopädischen Patienten die orale Antikoagulation an die Nierenfunktion anzupassen. In diesem Kapitel geht es um Apixaban und Niereninsuffizienz, okulopharyngeale Muskeldystrophie, Isolierung von Patienten, Schmerztherapie, Aspiration, Applikationsort und Wirkung von Medikamenten, Übelkeit (beim Arzt) sowie Verfügbarkeitsheuristik.

Bei einem erhöhten HZV ist der Effekt natürlich umgekehrt. Hier führt der hohe Blutfluss zu niedrigeren Plasmaspiegeln und zu einer raschen Umverteilung in die verschiedenen Kompartimente. Dieses Phänomen ist eine Erklärung, warum aufgeregte Patienten (und unaufgeregte Kinder) auf das Körpergewicht bezogen eine höhere Dosis benötigen, um einen gewünschten pharmakologischen Effekt zu erreichen.
