Zusammenfassung
„Dreckserkrankung!“ Herr May wollte eigentlich laut fluchen, aber dafür war die Luft zu knapp. Er stand mit aufgestützten Armen am Küchentisch und keuchte. Sein Asthmaspray hatte er leergepumpt und sich dann eine Mentholzigarette angesteckt. „Damit werde ich so alt wie Helmut Schmidt!“ kommentierte er stets die Vorwürfe von Freunden. In der Tat hatte ihm der Mentholdampf schon manchmal Erleichterung verschafft – aber nicht heute. In diesem Kapitel geht es um Asthma, Zeitkonstante, ECMO, Medikamentenüberhang, Neuromonitoring, Ketamin sowie Safety-I versus Safety-II.

Die Evolution hat sich noch andere Modelle ausgedacht. In der menschlichen Lunge ruht die Luft in der Alveole, die Kiemenatmung benutzt hingegen das Gegenstromprinzip : Blut und Wasser bewegen sich in gegengesetzter Richtung, sodass auch noch sehr kleine Partialdruckdifferenzen für die Sauerstoffaufnahme genutzt werden können. Bei den Tieren mit dem höchsten Sauerstoffbedarf (Vögel) findet der Gasaustausch nach dem Querstromprinzip statt: Das Blut fließt quer zum Luftstrom. Dieser strömt zusätzlich unidirektional und die Tiere können sehr hohe paO2-Werte erreichen (Farmer 2015). Ein Nebeneffekt des unidirektionalen Luftstroms ist, dass Vögel gleichzeitig ein- und ausatmend können. Kein Wunder also, dass Singvögel bei ihrem Konzert nicht kurzatmig werden und keine Pausen einlegen müssen.
