25 Jahre war die Bypassoperation bereits her. Herr Berthold war damals erst 45 Jahre alt gewesen und hatte nicht gedacht, noch so viele Jahre zu haben. In letzter Zeit war er zwar nicht mehr so fit, aber er war auch bereits 70 Jahre alt. Mit Ängsten war er zu seinem Hausarzt gegangen, aber der hatte ihn wegen des Herzens beruhigt. „Ihre regelmäßigen kardiologischen Kontrollen waren immer unauffällig!“ Als Grund für die Schwäche fand er eine Eisenmangelanämie – verursacht durch ständigen Blutverlust wegen eines Tumors im Dickdarm. Nach Prüfung aller Befunde kam nur eine Operation in Frage. Herr Berthold war ein fatalistischer Mensch, der die Dinge nahm, wie sie kamen. „Herr Doktor, wenn Sie das sagen, dann machen wir das“, meinte er. In diesem Kapitel geht es um diastolische Funktionsstörungen, T-Wellen-Morphologie, Kammerflimmern, kardialen Schock, die AaDO2, akute Herzinsuffizienz, Acetylsalicylsäure, den CHA2DS2-VASc-Score, den HAS-BLED-Score sowie Read-back.

Für diejenigen, die es genau wissen wollen: Treten T-Wellenveränderungen bei einer normalen Depolarisation auf, werden sie als primäre Repolarisationsstörungen bezeichnet. Ist bereits die Depolarisation nicht normal (z. B. in Folge eines Blockbildes), dann werden sie als sekundäre Repolarisationsstörungen interpretiert.

RQ ist ein Maß dafür, welche Substrate für die Energiegewinnung verstoffwechselt werden. Er beträgt bei der reinen Verstoffwechslung von Kohlenhydraten 1, bei Fetten 0,7 und bei Proteinen 0,81. Werte >1 finden sich bei schwerer körperlicher Belastung oder zu geringem Sauerstoffangebot mit anaerober Energiegewinnung. Zur Pufferung der metabolischen Azidose wird dann die renale Bikarbonatrückresorption erhöht, die sich entsprechend auf den RQ auswirkt.

Im Zusammenhand mit den CPOE ist beispielsweise die Vernetzung der Endgeräte (Perfusoren, Infusomaten, etc.) mit der elektronischen Anordnung zu nennen. Zurzeit gibt es dort noch erhebliche Sicherheitsbedenken, was einen möglichen Zugriff von außen betrifft. Im Vergleich dazu scheint eine einfache Medikamentenverwechslung trivial.
