Zusammenfassung
Nach der Erstzulassung des Medikamentes Xigris im Jahr 2001 hat es über etwa 10 Jahre eine heftige, z. T. auch hitzige Debatte gegeben. Hierbei standen sich eine nachvollziehbare, pathophysiologische Rationale sowie Meinungsverschiedenheiten zum Grad der Evidenz bzw. sich daraus ergebenden Unstimmigkeiten im Zulassungsverfahren diametral gegenüber. Schließlich waren die internationalen Kostenträger plötzlich mit einem bis dato unbekannt hohen Preisniveau für einmalige Therapieverfahren in der Intensivmedizin konfrontiert, die zu weiteren Diskussionen über eine Einbindung in gängige Standards der Sepsistherapie geführt haben. Obwohl das Medikament letztendlich vom Markt genommen wurde, werden hier einige Kernpunkte noch einmal reflektiert, da sie auf die äußerst schwierige Ausgangslage zur Neuentwicklung von Medikamenten in der Intensivmedizin verweisen, die durch ein hoch komplexes Krankheitsgeschehen sowie eine sehr inhomogene Patientenklientel maßgeblich mitbestimmt wird.
