Abstract

With improved components and a better understanding of the principles (Behrens and Searls 1986) that govern their safe and effective use, external fixators have become indispensable tools in the hands of the experienced trauma surgeon. The primary indications for external fixation are the stabilization of severe open fractures and infected non-unions, and the correction of both extremity malalignments and length discrepancies. Other indications include the initial stabilization of soft tissue and bony disruption in polytrauma patients, closed fractures with associated severe soft tissue injuries (crushing of the soft tissue sleeve, burns, dermatological conditions), severely comminuted diaphyseal and periarticular lesions, temporary transarticular stabilization of severe soft tissue and ligamentous injuries, certain pelvic ring disruptions, some children’s fractures, arthrodeses and osteotomies.
