Somatoforme Störungen: Konzeptuelle und methodologische Kritik und ein Plädoyer für die funktionale Analyse des Krankheitsverhaltens

Zusammenfassung

Körperliche Beschwerden ohne ausreichende körperliche Erklärungsmöglichkeiten bilden ein überdauerndes Thema der Fachliteratur, und die weitgehende Rat- und Hilflosigkeit der Fachwelt sind offenkundig. Dieser Sachverhalt ist an der breiten Auswahl unspezifischer und polypragmatischer Behandlungsangebote und an den terminologischen Moden zu erkennen. So ist auch der neuen Nomenklatur „somatoforme Störung“ und „Somatisierungsstörung“ mit großer Skepsis zu begegnen. Es sind kritische Prüfungen angezeigt, ob hier tatsächliche, empirisch begründete Fortschritte aufweisbar sind. Haben die an der Revision der Manuale DSM und ICD beteiligten Arbeitsgruppen vielleicht nur alte Konzepte wie Neurasthenie oder vegetative Dystonie unter anderem Namen wiederentdeckt? Ein aktuelles Beispiel solcher Redundanzen ist das chronische Erschöpfungssyndrom (CES). Welchen Anforderungen hinsichtlich Forschungslogik und repräsentativer empirischer Basis sollten die propagierten Vorschläge genügen? Ist neben psychiatrischen Vorstellungen der Stand der anderen, hier relevanten Forschungsrichtungen angemessen berücksichtigt?
