Zusammenfassung

Die Endoskopie des unteren Harntraktes sollte immer als Urethrozystoskopie durchgeführt werden und erlaubt damit die Befunddokumentation der gesamten Harnröhre und der Blase. Noch bis vor wenigen Jahren wurde von vielen Urologen angenommen, daß der endoskopische Befund des unteren Harntraktes ein Maß für die Schwere der Erkrankung, den Obstruktionsgrad infolge von BPH bzw. den potentiellen Erfolg einer Behandlung ist. Sucht man nach entsprechenden Informationen in der Literatur, so zeigt sich, daß nur sehr wenige gut validierte Daten über Sensitivität oder Spezifität bzw. den prädiktiven Wert der Urethrozystoskopie bei BPH existieren. So ist es nicht verwunderlich, daß die anläßlich der 3rd International Consultation on BPH befragten Experten nur zu 3% der Meinung waren, daß die Endoskopie die Obstruktion bei BPH definieren kann und daß 81% der Anwesenden dieser Untersuchung keinerlei diagnostischen Wert bei BPH zuordneten.[11]. Letztlich hat auch der Nachweis einer fehlenden Korrelation zwischen Symptomatik oder Prostatagröße mit der urodynamisch nachweisbaren Obstruktion zu einer neuen Definition der Aussagefähigkeit der Urethrozystoskopie bei BPH geführt.
