Zusammenfassung
Wie bei vielen gutartigen Erkrankungen bestimmt die Symptomatik auch bei der benignen Prostatahyperplasie (BPH) in entscheidender Weise Indikation, Art und Umfang einer rationalen Therapie. Zahl, Schweregrad und Verlauf einzelner Symptome sollen dem Untersucher die Krankheitsbewertung vom Patientenstandpunkt aus verdeutlichen (subjektiver Krankheitswert) und zusammen mit Untersuchungsbefunden (objektiver Krankheitswert) eine Abgrenzung der BPH zu anderen Krankheitsbildern erleichtern und eine Therapiefindung ermöglichen. Obwohl die BPH auch oligosymptomatisch oder selten asymptomatisch zu schweren Komplikationen führen kann (z. B. Infektion, Schädigung des oberen Harntrakts, Urämie), suchen die meisten BPH-Patienten den Arzt wegen störender Symptome auf. Die Wahl des Therapiekonzepts basiert dabei auf der individuellen Erwartungshaltung des Patienten (Verbesserung der Symptome) gegen eine Risikoabwägung (unbehandelte Krankheitsbefunde vs. Behandlungsrisiko) und letztlich einer Kostenabwägung. Das Spektrum der von Patienten mit klinischer BPH angegebenen Symptome ist breit und wird im allgemeinen in 2 Gruppen getrennt: obstruktive und irritative Symptome (Tabelle 4.1). Beide Symptomenkomplexe sind nicht BPH-spezifisch, sondern finden sich auch bei anderen Pathologien des unteren Harntrakts (traumatisch, infektiös, postoperativ) oder neurologischen Erkrankungen (z.B. Encephalitis disseminata, Morbus Parkinson oder Apoplex) und letztlich funktionellen Zuständen mit vermehrter Urinproduktion oder verminderter Speicherfunktion.
