Zusammenfassung

Von den 4 zum Herzen gehörenden Klappen kommt bezüglich der Erkrankungshäufigkeit und damit der Notwendigkeit einer chirurgischen Therapie der Aorten- und Mitralklappe die wichtigste Bedeutung zu. Erworbene Klappenerkrankungen betreffen überwiegend das linke Herz und sind meist rheumatischer Ursache. Die pathologischen Veränderungen mit Entstehung einer Stenose, Insuffizienz oder deren Kombination führen zu entsprechenden hämodynamischen Konsequenzen und klinischen Symptomen. Während Klappenstenosen eine Druckbelastung des vor der Stenose gelegenen Herzabschnitts bewirken, führen Klappeninsuffizienzen zu einer Volumenbelastung der vor und hinter dem Klappendefekt gelegenen Herzanteile. Grundsätzlich wird eine Volumenbelastung besser toleriert, was sich in einer längeren Lebenserwartung äußert.
