Zusammenfassung
Schwere Humeruskopffrakturen mit starker Fragmentdislokation führen unreponiert zu schlechten funktionellen Ergebnissen [5, 9, 13, 14]. Die Verbesserung der Fragmentstellung ist somit unumgänglich, wenngleich bei den Viersegmentfrakturen die Gefahr besteht, dass durch eine offene Reposition das frakturbedingte Nekroserisiko weiter erhöht wird. Nach Hägg et al. erhöht sich durch eine offene Reposition das Nekroserisiko um das Doppelte gegenüber einer geschlossenen Reposition [3]. Außerdem steigt das Nekroserisiko durch eine Frakturexposition weiter an, wie sie für eine Plattenosteosynthese erforderlich ist [4, 5, 7]. Neer empfiehlt daher für die Dreisegmentfrakturen die Minimal-Osteosynthese und für die Viersegmentfrakturen die Implantation einer Prothese [9].
